While FOL does not require
surveys or questionnaires, often schools and districts want to gather responses from
stakeholders. A number of free or relatively low-cost solutions are
available to both help you build appropriately valid and reliable
questions but also to tabulate and present results.

National Study of School
Evaluation (NSSE) provides educational leaders with
state-of-the-art resources, tools, and support services to enhance and
promote student growth and school improvement through accreditation. Both
paper and web-based surveys are available for a fee. This is a non-profit
educational research and development organization founded in 1933 by the
six regional school
accreditation commissions in the United States.

Survey Monkey
says it's single purpose is "to enable anyone to create professional
online surveys quickly and easily." They'll do a 10 question with no
more than 100 responses for free.

Matt's Script
Archive has free CGI scripts that will enable you to do surveys through your own
webpage.

EForm Easy to use, and their Tech Support
is good. Download a trial version. The program will allow you to create the form, and then e-mail it to
selected recipients or put the form on a web page or both. The responses come by e-mail to an account that should be used for survey results only
(although you can manage multiple surveys through one address). Responses are compiled in an Access database. The program will generate
some graphs and summaries, or you can do your own by exporting the data to
Excel (or whatever).

which
can be used to collect data for a multitude of purposes.

Infopoll offers online survey software and services.

SurveySolutions,
winner of PC Magazine's Editors' Choice Award, provides
everything you need to implement a successful Internet
survey at relatively low cost (see their educational discounts). Perseus
is giving away 100,000 free copies of SurveySolutions for the Web for use in classrooms grade twelve and
under. Download a trial version.

QuestionPro offers web based software for creating and distributing
surveys. It consists of an intuitive wizard interface for creating survey
questions, tools for distributing your survey via email or your website, and
tools for analyzing and viewing your results. You simply build your survey
and email it to a list of potential respondents. QuestionPro will take care
collecting and recording the responses. Results are available in real time.

Perceptions was founded to help educational
leaders gather and analyze the data necessary to plan and improve. Many
student, staff, parent/community, and program/services surveys are
available at a cost.
